We spend at least 2 days per week in Moonee Valley for one of the following reasons:*

Please attach a payslip or a letter on company letterhead from your employer, specifically stating your work address, the number of days you work in the municipality, and confirming your employment.
Please attach a letter confirming enrolment from a kindergarten, school, university, or TAFE (outside of CRS), including the address and details on the days attending.
Please attach a childcare statement, receipt, or Childcare Benefit letter from Centrelink, specifying the days your child is in care in Moonee Valley and the childcare facility's address.

Please attach a statutory declaration and a recent gas/ electricity/ water bill from your informal care provider, indicating the days of access in Moonee Valley and the address where care is provided.

However, families residing outside the municipality can still apply. Please note applications will not be processed in the initial round of offers if they do not meet one of the criteria.

The personal information (information) requested on this form is being collected by Moonee Valley City Council (Council) for the purpose of Kindergarten applications. This information will be stored in Council’s customer database and will be handled in accordance with the Privacy and Data Protection Act 2014 (Vic), Council’s Information Privacy & Security Policy, and the Health Records Act 2001 (Vic), and will be accessed by authorised employees of the enrolment process. This information will be used to communicate with the applicant for the enrolment in and delivery of kindergarten services. Council will not disclose the information without your consent, except where required to do so by law. If you do not provide the requested information, the application/request may not be able to be processed.
