  • To register your child for kindergarten please complete this form. There is no cost to register. Please submit only ONE registration form per child.

    To complete this form, you’ll need:

    • Evidence of your residential address like a driver’s licence, rates notice, rental agreement or recent utility bill
    • Concession / Health Care or Pensioner card details (if applicable)

    No Jab No Play (Immunisation)

    To attend kindergarten, you’ll need to provide the kindergarten with your child’s Australian Immunisation Register History Statement when you enrol. For more information, visit ‘No Jab No Play.’

  • HOW TO USE THIS FORM

    If a family needs support to register their child for kindergarten, a support worker or practitioner can complete a referral form with the family.

    2. Explain that any information collected is for the purposes of supporting them to register their children for kindergarten and as outlined in the collection statement below. The information will not be shared for any other purpose

    PRIVACY COLLECTION STATEMENT

    The personal and health information you provide when completing this kindergarten registration is being collected by the Mornington Peninsula Shire (the Shire) in accordance with the Privacy and Data Protection Act 2014, the Health Records Act 2001 and the Shire’s Privacy Policy. The information will be used to facilitate the kindergarten application and allocation process. The information will be used for this purpose and shared with kindergartens participating in the Mornington Peninsula Central Registration Scheme and also for the following purposes;

    • to plan, fund, implement, monitor, regulate and evaluate the Mornington Peninsula Shire Kindergarten Central Registration service and functions
    • to fulfil statutory and other legal functions and duties
    • to comply with reporting requirements
    • to inform service delivery between the Early Years and Child and Family Health

    Services including to help predict future needs and to improve services. The information may also be used for a purpose that you would reasonably expect or where the law requires the Shire to do so. You may access the personal and health information you have provided or make changes to the information if it is incorrect by contacting the Shire.       • Three and four-year-old kindergarten places are allocated separately. Every effort will be made to place my child at the same service for four-year-old kindergarten but this is not guaranteed.

      • I can only access one funded kindergarten place per year.
      • My child is only able to access two years of funded kindergarten in total, unless my child is assessed as being eligible for a second year of Three-Year Old or Four-Year-Old Kindergarten by their Early Childhood Teacher.
