Park Orchards Kindergarten Enrolment Application Form
If you would like to place your child on the waitlist at Park Orchards Kindergarten, please complete the following form. Applications can be made from your child’s second birthday.

Please note, to qualify for the 3 year old program, your child must turn three years of age on or before April 30th in the year of attendance. To qualify for the 4 year old program, your child must turn four years of age on or before April 30th in the year of attendance.

Completing an application form does not guarantee a position in the program. Where the number of applicants exceeds the number of places available, the following criteria and process will apply.
- Children who have received funding for a second year of kindergarten
- Children who were eligible to attend in the previous year but deferred or withdrew from the service prior to the April data collection.
- Children who attended the 3 year old program at Park Orchards Kindergarten and are looking to attend the 4 year old program.
- Children who were on the waitlist for the 3 year old program at Park Orchards Kindergarten and are looking to attend the 4 year old program.
- Children who fall under Government Priority of Access criteria
  - Known to Child Protection
  - At risk of abuse or neglect
  - Aboriginal or Torres Strait Islander
  - Asylum seeker or Refugee
  - Has additional needs in order to fully participate in the program
    
    Identified specific disability or developmental delay
    
    Requires a combination of services which are individually planned
    
    Has a referral form from NDIS, ECIS, PSFO or MCHN
  - Multiple births (triplets or more)
- Children who live within postcode 3114 (primary residential address only).
- Children who have had a sibling attend the kindergarten previously.
- Children who do not fit any of the above criteria, but were enrolled on time.
- Late applications, according to date the application was received.
Where the number of applications exceeds the number of places available, children with the same priority will be electronically balloted.

The Committee of Management of Park Orchards Kindergarten has opted into the State Government’s “Free Kinder” intiative, so no fees are payable for attending our 3 and 4 year programs (15 hours each week). At times, a one-off fee may be charged for a special event (e.g. excursion). Further information about Free Kinder can be read here - https://www.vic.gov.au/kinder-quick-guide-parents

This enrolment application form requires you to:
- Pay a non refundable application fee payment of $50.00. This payment can be made via credit card at the end of the form
- Attach a copy of your child’s birth certificate
- Attach a copy of your child’s immunisation status (formal statement from Medicare or signed medical exemption).
- Proof of address (e.g. copy of gas / electricity bill, rates notice, etc.)
Please note you will need to complete a separate application form for each child.
